Each carrier has a method to port out your number and each process starts out identically:

First, you need the number you are porting still active with the carrier. If you are taking advantage of a trade-in promo, the request needs to be made prior to trading in.

You need to use the carrier-supported method to obtain a Number Transfer PIN
With US Mobile, your PIN is what is required to add usage to your account. Regardless, you may need to call US Mobile at 1-888-848-1788 or email help@usmobile.com for your account number, they may be able to help you obtain the PIN too.
